Report a TPA injury
When an injury occurs, report the injury to SFM Risk Solutions as soon as possible —ideally, within 24 hours.
Notice: On Minnesota claims, we are required to notify you that a person who files a claim with intent to defraud or helps commit a fraud against an insurer is guilty of a crime.
Report an injury through CompOnline
If you’ve registered for CompOnline®, log in and save time completing the first report of injury there. Reporting through CompOnline is the most convenient way to begin the claims process.
Report online
If you don’t have access to CompOnline, fill out the first report of injury form online. All you need is your SFM Risk Solutions policy number and the date of injury to get started.
Other ways to report
If you cannot report online, you have two other ways to report injuries to your TPA claims representative.
- Report by fax
Complete your state’s first report of injury form and fax it to (800) 944-1169. Keep your fax confirmation sheet as evidence that you reported on time. - Report by phone
Call (800) WC-CLAIM [922-5246] to report an injury by phone.
You’ll receive contact within 24 hours of reporting a claim. Our claims adjusters promptly handle, thoroughly investigate and fairly resolve your claims.
